	Enroll for only $19.95 per year
	After enrolling, you will receive a membership kit in the mail including your membership number, a letter for your doctor to sign, a medical questionnaire, other forms, and instructions on how to send in your first order.
	Your membership kit will also contain your MedAdvantage card so you can start saving money at US drug stores immediately.
	Make an appointment with your doctor to review all of your prescriptions and complete the Doctor's medical form.
	Once you have finished filling out the forms in your membership kit, have your doctor fax them to us from your doctor's office or mail us the originals. It is against the law for us to accept copies of prescriptions by mail or faxed copies from anywhere other than your doctor's office. At this time, you will also send us your payment information (credit card number, etc.)
	From time of dispensing, your prescriptions will arrive via XpressPost/USA in 5 to 10 business days, under normal circumstances.

Cancellation Policy
You have 30 days in which to cancel your membership in this program, unless your medical file has been activated, which occurs when you send us the completed enrollment kit and prescription(s) by mail or by fax from your doctor's office.

Once medication orders have been shipped, there is NO REFUND or EXCHANGE.
